   Signed Broker-Carrier Agreement

   Our standard agreement. Lays out rates, payment terms, claims process, and operational expectations. We'll send a fresh copy on request, or you can pull the template from your factor if it's compatible.
2. 02
   MC and DOT authority

   Active operating authority confirmed via FMCSA SAFER. Your safety rating and insurance status are pulled automatically — no need to send screenshots.
3. 03
   Certificate of Insurance

   Valid COI with LASLINT named as additional insured for auto liability ($1M minimum) and cargo ($100K minimum, higher for heavy haul). Direct from your agent — broker email or fax both work.
4. 04
   W9

   Standard IRS W9 for payment processing. EIN preferred over SSN for owner-operators with EINs.
5. 05
   Banking / ACH info

   Voided check or a signed ACH form with your bank routing and account number. We pay via ACH only — no paper checks.

Our review — what we look for.

• Active MC and DOT authority with no recent revocations or fraud flags
• Satisfactory safety rating, or Conditional with documented remediation
• Insurance limits meeting our minimums and certificate not within 30 days of expiry
• No active double-broker complaints in industry watchlists (FreightWaves, Carrier411, etc.)
• Driver count and equipment fleet aligned with the lanes you're asking to run

If any of these don't pass, we'll tell you exactly why and what would unlock the relationship. We don't ghost — we'd rather lose a load than string a carrier along.
